Apply to Frog Pond
We welcome applications for children ages 16 months through 5 years, 11 months. Families may apply before or after touring the center.
1. Complete the online student application below.
2. Submit the non-refundable $200 application fee. The application and fee place your child on our waiting list but do not guarantee enrollment.
3. We will review classroom availability based on your child’s age, requested schedule, and desired start date, then contact you regarding placement.
4. Once placement and a start date are confirmed, you will receive a Brightwheel invitation to complete emergency contact, health, medical, and authorization forms. All required documents are due at least two weeks before your child’s first day.

Physical Examination
Each child is required to have a physical examination by or under the direction of a physician before enrollment or within one month after the first day of attendance. If the child has had a physical exam prior to attendance, it must meet the following requirements:
For children under 18 months, within three months prior to attendance.
For children aged 19 months through 24 months, within six months prior to attendance
For children two through five years of age, within 12 months prior to attendance.
We must have on file documentation of all age-appropriate immunizations before a child begins at the school. Without appropriate documentation, children without documentation of immunization will not be permitted to attend in accordance with regulations of the State Board of Health.
Withdrawal
Families are asked to provide at least 90 days’ written notice before withdrawing from the program. If Frog Pond determines that we can no longer safely or appropriately meet a child’s needs, we will work with the family to discuss supports, next steps, and a transition plan whenever circumstances allow.
